Good Faith Estimate

• $200-$250
(All Sessions are 45 minutes.)
Supervision
• $300 per month for 4 hours in a group setting

Insurance
I am in network with the following insurance carriers:
- Aetna
- BlueCross/Blue Shield
- United Healthcare
- Cigna and Evernorth
- UnitedHealthcare UHC/UBH
- Optum
If your insurance is not listed, I would be considered out-of-network with your company. Most mental health insurance plans cover the services offered by our licensed therapist as “out-of-network” benefits. We can provide you with a superbill after you receive services. A superbill is a specialized invoice that includes the procedure coding, diagnosis coding, and session dates that your insurance carrier will need to reimburse you.
We suggest calling your insurance company in advance to inquire about coverage for the service you are looking for. Please carefully check your out-of-network benefit coverage by asking your insurance carrier the following questions:
• Do I have mental health insurance benefits?
• What is my deductible, and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage required from my primary care physician?
Reduced Fee
Reduced-fee services are available on a limited basis.
Cancellation Policy
Notice of 24 hours is required for all canceled or rescheduled appointments. Otherwise, you will be charged the full cost of the session.
